GDPR Consent for Clinical Assessment

Neurocheckpro Limited. This document explains how we process your health data and your right to withdraw consent.

This is a specific request from Neurocheckpro Limited for your consent to the processing of certain categories of personal data under the UK General Data Protection Regulation (UK GDPR) and Data Protection Act 2018. Under Article 9 of UK GDPR, personal data relating to health is classed as a "special category" and requires your explicit consent.

In order to provide our clinical assessment services to you, we may collect the following health data:

• Details of your current complaint and its history; • Your psychiatric history and current treatment; • Family history of physical and mental illness; • Medication history and current medications; • Social history including accommodation and employment circumstances; • Risk history (e.g. self-harm, risk from or to others); • Personality, lifestyle, hobbies, and interests; • Your current mental state.

How We Use Your Data

We will process the data we collect for the following purposes:

• To register you for our clinical assessments and services; • To manage and maintain your details with us; • To provide clinical assessments and a diagnosis; • To monitor your progress and evaluate the impact of our services; • To provide a treatment plan, where necessary; • To provide reports to other relevant health professionals involved in your care (e.g. your GP or other clinicians); • To research and understand how we can improve support for individuals and their families.

Sharing Your Data

We will not share your health information with anyone else unless you give us further consent to do so, subject to applicable legal requirements or our safeguarding obligations, which may require us to share information in certain circumstances.

Your Right to Withdraw Consent

You have the right to withdraw the consent granted to the processing of your personal data relating to health at any time. To withdraw consent, please email us at support@neurocheckpro.com.

Withdrawing consent does not affect the lawfulness of processing carried out before the withdrawal.
